VBS 2010 Medical Release/Registration
August 2nd-6th
9AM-12:30PM
Kids, come to SonQuest Rain Forest VBS where you will learn to follow Jesus on a life changing adventure!
VBS is for all kids who are 4 years-6th grade.The fee is only $10 plus $5 for each additional child. The fee includes daily snacks, crafts, activities, a unique VBS T-shirt and a CD with all the great VBS songs.
Please fill out the registration/medical release form and return to Donna Tucker as soon as possible! The completed form may also be faxed to the office.
